New Patient Registration

Please note that due to the rising number of patients moving into the area the practice are currently restricting the number of patients who may register each week. This restriction has been necessary to ensure that our patients continue to receive a good service from the practice. Registrations open each day at 12 noon and patients will be registered at that point until the quota is filled.  This quota does not include new babies and relatives of existing patients who live in the practice area who may register with the practice at any time.

New patients registering with the practice will be asked to complete a health questionnaire. The practice does not discriminate on the grounds of: sex, race, colour, nationality, ethnic origin, marital status, age, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition. 

You must also produce proof of your identity (passport or driver’s license) and proof of residency dated within the last 3 months (eg utility bill) in the UK.

If you are not a permanent resident of the UK you may be asked to produce other documentation.

Practice Area 

The practice area, partially covers the following post codes: EH 3, 10, 11, 12, 13, 14.

Change of Details

Please advise the reception staff should you change your name, address, telephone number or any other personal information recorded in your case notes. Remember to also inform the hospital if you’re waiting for an appointment or having treatment. If you move house out-with the practice area you should register with another GP as soon as you move.
