Online 'Pre-Registration' With The Practice

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. You will also need to produce proof of identity at that visit. Registrations open each day at 12 noon and patients will be registered at that point until the quota is filled.  This includes registration of patients who have completed the pre-registration form. Please note that you will not be able to book appointments until after the registration process is complete.  Online registration forms will be kept by the Practice for 2 weeks after which time they will be destroyed if the registration process has not been completed.

Registration of children under 16 will only be accepted if the parent/guardian is either registering at the same time or if they are already registered at the practice.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
